Registration: 

Please fill in and return the registration form to ESH, preferably by e-mail celine.caudroit@univ-paris-diderot.fr or register online.


Participants are warmly invited to indicate if they would like to register for the “Onsite Dinner Package” which includes two evening meals with the Chairs and Speakers on September 18 and 19 and an invitation to attend the Gala Dinner on September 20, 2008 or if they prefer to organize their own evening meals during the conference.
Please note that if you chose the “Onsite Dinner Package”, it is necessary to register and pay for your Package fifteen (15) days before the start of the conference (ESH regrets that no onsite registrations can be accepted).

Registration Fee: 560 euros (The registration fee includes the teaching material and lunch)
or
Registration and Dinner Package: 690 Euros
Your seat in the conference will be definitively reserved upon receipt of your fee.
Payments for the “Onsite Dinner Package” may be made by cheque or bank transfer before September 4, 2008.

ESH Scholarships:
A limited number of ESH scholarships are also available to scientists and clinicians.
In order to apply please fill the application form and send to ESH with your:
1) Curriculum Vitae*, 
2) Letter of motivation*, 
3) Abstract*,
4) List of publications.

*Mandatory documents. If not submitted, the application cannot be taken in consideration.

Deadline for scholarships is closed.

CANCELLATION POLICY OF REGISTRATION FEE
In case of cancellation, up until one month prior to the beginning of the meeting, your registration fee will be reimbursed in full (less bank charges).
If the cancellation is notified between 1 and 30 days prior to the beginning of the meeting, 50% of the registration fee will be reimbursed (less bank charges).
Cancellations notified during the meeting will not be reimbursed.

Please note that ESH can accept VISA, MASTERCARD/EUROCARD and CB.
